What if our community’s “helpers” could spend less time searching and more time serving people? They are now able to do just that, thanks to the Saginaw Information System (SIS).
SCHD is coordinating the work on SIS, a new electronic platform built to help community health workers, hospitals, doctor’s offices, social services providers, case managers, and other community-based organizations (CBOs) connect residents to the right services – faster and more efficiently than ever before. PartnerShift Network is the CBO champion.
Fully integrated with 211, SIS provides a single point of entry for better incoming and outgoing referrals – without redundancies, duplication of effort or unnecessary obstacles – while providing a 360 feedback loop back to the referring organization.
It provides one-stop shopping to match individuals to services they need with real-time access to what is available and what they qualify for. It tracks referrals so no one falls through the cracks. And, it uses data and shared insights to improve outcomes and strengthen community partnerships.
Early adopters of SIS include SCHD, Covenant, United Way of Saginaw County, Saginaw Community Mental Health Authority, Saginaw Community Food Club & Kitchen, Saginaw County Commission on Aging, PartnerShift Network, Saginaw County Community Action Center, The Salvation Army, and Disability Network of Mid-Michigan.
